CLINICAL EVALUATION

CARE COORDINATION

Active support to ensure care plans are implemented effectively, even in the most complex situations. Ongoing care is often provided by an advanced practice provider (NP or PA) under Dr. Sharon’s direct supervision, ensuring both accessibility and expert oversight.

 

We collaborate closely with extended care teams — including primary care providers, subspecialists, therapists, and schools — to maintain continuity, address barriers, and coordinate logistics for testing, treatments, and follow-up appointments. 

 

Our team is experienced in prescribing and providing sophisticated therapies such as intravenous immunoglobulin (IVIG) and biologic agents, always tailored to clinical indication and goals.

A 45–60 minute encounter with Dr. Sharon, conducted via telehealth (THV) or in person depending on preference and clinical requirements. These visits include a detailed history, multidisciplinary assessment, and integration of findings. They are typically the first step for new patients but are also recommended for those not seen in over a year or with significant changes in health. 

 

Encounters are supplemented with a clear, actionable next steps for evaluation and treatment, with referrals to appropriate external experts when needed. Dr. Sharon has extensive expertise in creating effective treatment plans for complicated presentations, including non-pharmacologic strategies, lifestyle modifications, supportive therapies, and advanced treatments.

NICC offers concierge style service packages, that are best suitable for patients with complex, chronic, and/or debilitating conditions, and meet their unique needs.  

EXPEDITED DIAGNOSTIC EVALUATION
[Intake for new patients]

Comprehensive evaluation for new referrals that integrates both clinical encounter (in-person or telehealth) with coordinated follow-up care for prompt diagnosis and timely treatment. This service offers an extensive assessment, initial diagnostic recommendations, and a management plan or evaluation through 2 visits with Dr. Sharon and care coordination within 30 days.

This package is aimed at patients experiencing persistent, unexplained symptoms that lack a diagnosis, or those with a known diagnosis who need an effective treatment plan, that can be provided through out Longitudinal Management Care plan.

LONGITUDINAL CARE MANAGEMENT
[Concierge for established patients]

Ongoing care coordination by the Longmont team under Dr. Sharon’s direct supervision, ideal for complex patients, including those receiving IVIG, biologic infusions, or other advanced therapies. Enrollment follows a comprehensive evaluation and is reviewed every 3–4 months.

SECOND OPINION /
Outpatient Consultation

Focused, clinical encounters with Dr. Sharon to address a specific health concern and offer expert recommendations for next steps, without ongoing coordination or follow-up.

PAY STRUCTURE

Clinical services at NICC include clinical encounters with Dr. Sharon and care coordination by our Longmont-based team. Charges for visits with Dr. Sharon are based on time, whether in person or via telehealth.
